Is it possible to customize the title tag on the forum index page?

By default it says (translated from Spanish) 'Categories - Forum Name'

Reason I'm asking is that from an SEO perspective the index page is the most important page (usually).

yes, when using JoomSEF, you can customize page title on any page that is processed by JoomSEF.
Just locate and edit it in JoomSEF URL editor/viewer.

in which case what is the Real URL or SEF URL of that page?

yes, even the index page.
SEO URL for homepage is simply empty string "". Choose the "homepage" choice from the select box at the editor page to edit it.

It is probably not correct, as you should not see 0 hits for homepage.
Maybe itemId is missing there. E.g. real URL for our site is "index.php?Itemid=1".

Maybe you did not clean your cache after doing the change.
But in general, this should work fine this way.
