to tell the reason of the duplicate URL, you should check what is the originial "non-seo" URL stored in DB.
Comparing the differences should give some hints about why there exists 2 links to same article.

Based on the SEO version of the links, it appears you have the same article organized in multiple categories or that there exist multiple paths to the article.
