global - se friendly urls to no

uninstalled and reinstalled other plugin(JCE) all ok

Then there are basically 2 possibilities:

1. there is some error in your .htaccess file (typo or unsupported directive) or
2. usage of .htaccess files is prohibited by global web server configuration

I think more likely is the option #1.
It is also possible that the mod_rewrite is not enabled on your server. You can check this in Joomla / Help / System Info, in PHP Information (check for apache2handler section or similar, where list of Apache module enable should be present)

we just received your .htaccess via email.

You have there a lot of extra directives that may not be allowed and thus be causing this 500 error.
Start with commenting them out and you will see which one is the reason.

E.g. comment out SecFilterEngine off, SecFilterEngine off.
Also wrong value or that usage of php_flag directive may be causing this. Depends if your server configuration allows overriding particular settings.

Anyway, your server administrator should be able to help you with this. We can also do that, but as this is related to your specific server configuration, not connected directly with JoomSEF, so adjustments there would be a paid support option.

I have attached the htaccess file, as mentioned everything else is working with no errors - other extensions have no problems installing

SecFilterEngine off, SecFilterEngine off are required by server. If i turn them off to install and then turn them back on will the ext still work

tried commenting out the below but error remains
Options +FollowSymLinks
SecFilterEngine off
SecFilterEngine off

Mystery to why this ext won't install and others do!
