I have followed the install for 1.4 to the letter on two stock standard 1.0.12 Joomla installations.
I get SEF URLS generated but they do not appear in the browser. I have other versions of SEF installed on other sites on the same server and they work fine. Here is an extract from the URL Manager. The counter increments fine.
5 links/index.php index.php?option=com_weblinks
Here is what appears in the browser for the above link:

Houston we seem to have a problem. Any help would be appreciated. Here is the .htaccess contents:
