Yes I created the fields in the Customer Fields Extra fields option. I finally got them working using a name value pair.
We wanted this:
How did you hear about us? |Text field box|
I tried many,many different configurations,nothing worked until I tried using only two words as shown below.
Referred By {REFERRED BY}
I wish your documentation actually said only a one or two word name/value pair would work because many of the existing marks in the template are short sentences with only one word as the mark. This does not seem to work in adding extra fields. Only one or two word matched pairs work.
