When the lines I posted are added to the .htaccess file, there should be errors reported instead of the blank page you talked about in the first post when you click the Generate XML button. Are there any errors reported, or just the blank page? If there is still just a blank page, you will probably need to discuss the error reporting options with your web host.
As regards the Google indexing - I'm sorry, but I don't know why Google doesn't index some of the pages, maybe it just needs more time.
