This means there was a problem executing the query on your database. Please, check that you have the jos_redirection table present in your database and the Joomla user has privileges to run the INSERT queries. If this doesn't help, try uninstalling JoomSEF, deleting jos_redirection table manually from database, and installing JoomSEF again.
