Do you use correct module? You must use the mod_falang, not Joomla's Language Switcher.

Are there duplicate empty SEF URLs generated in JoomSEF's URLs Manager differing only in the lang variable in their non-SEF URL?
Could you try language switching with generation of new SEF URLs enabled in JoomSEF?

Make sure that there are no duplicate empty SEF URLs generated and also clear Joomla's and JoomSEF's caches.

We'll also test latest FaLang to see if there's any incompatibility introduced and let you know.

Thanks, its working now, the problem we have now is that for the default language EN the url also adds /en for all pages, even if go to the home page directly by typing the url it will still add/en.

Yes, this is the way JoomSEF currently works. We already plan to implement an option not to add language code to URL for main language.
