Have you tried Purge URLs option in JoomSEF component administration? If you have some unnecessary custom URLs, you can delete them as well...

Yes I purge every single time, and I've turned off caching, just to make sure I'm seeing fresh.

I will try to reproduce the problem in my local Joomla installation and will let you know.

EDIT:
Virtuemart works good for me, URLs like /virtuemart/hand-tools/hammer.html are generated. Do you have some special settings of JoomSEF or Virtuemart that could cause this? Is there a possibility to start from scratch?

All default settings.
