Hello. We found it may be caused by bug in JoomSEF, which created "404" article with unspecified Language, which can cause problem on multilingual site as yours.
We fixed it by simply opening the "404" article and re-saving it again with "All languages".
We will fix it also in JoomSEF next version.

Side note: 404 Article must have Title exactly "404".

Hello. You can select menu item instead of text in JoomSEF Configuration -> 404 Page. There you can also edit article text.

I do not quite understand. You mean some menu is missing on 404 page? You need to create menu item (like 404) which contains 404 page and assign modules for it just like for any other menu item.

we have the same problem - the site is not making a 404.html page - every time a wrong link is enteretd it makes a new url - any solution here? - using joomsef4
