When
Show Category y
Show Section y
[Section]/[Category]/page.html -its ok

But when:
Show Category y
Show Section n

[Category]/page.html -20%
/page.html - 80% wrong link build

What does the Real non-SEF URL look like for the wrong links? You can find it in JoomSEF's URLs Manager.
What section and category are set for the articles with wrong links?

No direct link from menu. link to article only from other article

The only difference I can see is in the Itemid. What are the titles for your menu items with IDs 209 and 224? Could you please also check if the category ID 70 is correct? You can use Joomla's Menu Manager and Category Manager to check this.
